Two new Social Worker posts has been created within Mental Health and Learning Disability Services, as we embark on an ambitious program of change. Our vision is to reduce the reliance on residential care for adults with a learning disability or mental illness. We believe that wherever possible people should live in their own homes and be supported to live independent lives, within their communities.

The social workers (Progression and Review) will be expected to work in a team working alongside social workers in the, mental health and learning disability teams. Including commissioning, health, housing, and independent sector partners to support the delivery of this vision. The work will involve undertaking a programme of reviews for people in existing residential care placements, with the aim wherever possible of enabling people to step down safely. There will also be an element of right-sizing existing packages should that be necessary. You will be required to have experience of delivering services to people with complex needs.

A basic knowledge of Welsh is required to accomplish this post. Support can be provided on appointment to reach this level.

A Disclosure and Barring Service (DBS) check will be requested for this post.

As part of the Council’s Policy and Procedure it is now a requirement that employees must register with the DBS Update Service for which there is an annual subscription fee of £13 borne by the individual.  The Authority will pay for the initialDBS check.
