Carmarthenshire County Council is looking to appoint a motivated and experienced individual.
It is essential that applicants have experience of working in a demanding environment across Health and Social Care with the knowledge of community and hospital settings.
Possess the ability to work in partnership with a range of organisations and services to produce assessments and care and support plans that are person centered and outcome focused.
To be conversant with relevant legislation and statutory obligations and will need to be able to demonstrate an active commitment to the principles of promoting independence for people with disabilities.
The position is linked to the Teifi, Taf and Towy Community Resource Team covering adults with physical disabilities and older adults with cognitive difficulties.
A basic knowledge of Welsh is required to accomplish this post. Support can be provided on appointment to reach this level.

A Disclosure and Barring Service (DBS) check will be requested for this post.
As part of the Council’s Policy and Procedure it is now a requirement that employees must register with the DBS Update Service for which there is an annual subscription fee of £13 borne by the individual. The Authority will pay for the initial DBS check.
